چکیده

RESUSCITATION by cardiac massage through the closed chest has been described recently by Kouwenhoven and associates.1 This technic consists of applying firm, rhythmic pressure to the lower sternum, thus compressing the heart between it and the spine. Based on exhaustive animal experiments, this method was instrumental in the effective cardiac resuscitation of 20 patients reported by the authors. Three of these cases were observed to be in ventricular fibrillation. The present report concerns an episode of ventricular fibrillation during retrograde left heart catheterization that afforded an opportunity to obtain direct aortic pressure measurements while closed-chest cardiac compression was being performed.

Closed-chest resuscitation in a cardiac catheterization laboratory.

THE ABILIlTY to resuscitate a patient in cardiac standstill or ventricular fibrillation without opening the chest constitutes a major advance in the treatment of acute cardiac emergencies. In 1957, Rainer and Bullough' reported successful treatment of presumptive cardiac arrest during operation in children by flexing the legs and thighs against the closed chest. Kouwenhoven and co-workers2 in 1...

Editorial Chest Compression Rate

The first description of modern cardiopulmonary resuscitation (CPR) included the instruction to compress the chest “about 60 times per minute”1; however, the optimal compression rate was unknown. Franz Koenig is credited with describing the original technique for external cardiac massage, which included a compression rate of 30 to 40 per minute.2 But in the first published description of extern...

Aortic diameter and pressure-flow sequence identify mechanism of blood flow during external chest compression in dogs.

Aortic flow and pressure relations and aortic diameter were examined during sinus rhythm, internal cardiac massage, vest cardiopulmonary resuscitation, conventional manual cardiopulmonary resuscitation and high impulse manual cardiopulmonary resuscitation in 14 anesthetized large dogs. During sinus rhythm and during internal cardiac massage, ascending aortic flow and pressure increased simultan...
